What is a form conversion request?

A form conversion request is when you have an actual paper form and you want us to convert it for you so that you can use it with our forms application.

Each organization is entitled to 1 free conversion request, after that you can request a quote from our team to help you build even more forms. Form conversion charges are dependent on the complexity and size of the form, so you will be quoted on the time it will take to complete the form.

How to submit a form conversion request

There are two ways to do this. One being the preferred way, through the forms builder, and the second as an alternative, if you have already used the conversion request through the forms builder, as the limit is set to one.

Step one:

Click on your forms icon in the left side menu and click on "New Form".

Click on "Convert a Form".

Click "Upload" on the popup screen

Select your file and click "Open".

After the form is attached, click "Send".

Click "OK".

After you clicked "OK", you will be directed back to the forms builder for your new form. Since you won't be building a form at the time, you can hover over "Save & Close" and click "Close without saving".

Once you have done this you will be redirected to your forms list.

What happens after the form request is sent?

Once you have submitted your form request, our team reach out to you, then get started converting it into a mobile form for you.  When it's ready to be tested, a member of our team will reach out to you via email.
